According to the complaint, the incident dates back to November 2017 at the Office of the Director of Food and Drugs Administration (FDA), Bambolim, where the accused, Pradeep Volvoikar, allegedly prepared and submitted a forged and fabricated application to renew a Food Safety and Standards Act, 2006 certificate in the name of his deceased father, Prabhakar V.
