The applicant has to submit a medical certificate in the prescribed proforma issued by the concerned hospitals where the victim expired due to Covid -19. The medical report should have a reference of the RTPCR / RAT report of any authorised LAB OF ICMR.
It has come to the notice that the applicants applying for financial assistance under the said scheme are getting hardship for obtaining a death certificate from the concerned hospital.
